Community Assessment Survey

The purpose of Head Start Child and Family Services, Inc. Community Assessment, is to review relevant physical, economic and social data, in order to determine community insufficiencies and resources in our service area. The Community Assessment focuses on the needs of families that are enrolled in programs that our agency provides as well as citizens of the Crawford and Sebastian County Service Areas. In addition to federal age and income eligibility criteria for enrollment, our community assessment is the basis for our locally developed enrollment criteria, and a tool to utilize in the expansion and altering of the services we provide to the families in our community.

We encourage you to please take a minute and fill out one of the surveys below. Your answers will be used strictly for the development of programs in our agency.
